Photo Release
I agree to the Photo Release outlined below.
I hereby grant to the Holter Museum of Art the right to use and publish photographs of myself and/or my children, or in which we may be included, for website design, editorial, trade, merchandising display and advertising for the purpose of promoting the activities of Holter Museum of Art; to alter the same without restriction and to copyright the same. I hereby release Holter Museum of Art from all claims and liability relating to said photographs.
Liability Waiver*
I have carefully read and understand the following provisions. By checking this box, I hereby certify and acknowledge my understanding and agree to be legally bound by the terms and conditions set forth.
I assert that my participation in this Holter class, workshop, or event is voluntary and at my own risk. That the activities in this class can pose bodily harm if instructions and safety procedures are not followed on the part of participants.
With this knowledge, I here by release, waive, indemnify and save harmless the Holter Museum of Art and all Holter Museum employees, teachers, and associates from any liability for all claims, demands, and physical or otherwise injuries or harm suffered by my person or property arising out of or connected with my participation and/or use of any services, equipment or facilities provided by the Holter Museum of Art and/or Associates.
REFUND Policy*
I have carefully read this agreement and understand it to be an acknowledgement on my part of the Holter Museum of Art's refund policy.
Preregistration is required for all Holter educational programs. Full tuition will be refunded on any cancellation up to four weeks prior to beginning of class with the exception of a $14 processing fee, after that 40% of tuition will be refunded upon cancellation up to two weeks prior to beginning of class. Cancellation within two weeks of beginning of class will not be refunded.
